Obituary

Veteran reality show producer, Dave Forman died unexpectedly Tuesday morning, June 8th. He was 52 years old.

Dave Forman was a pioneer in the reality television genre; driven by a passion for all ?emergency agencies?; FDNY, LAPD, NYPD, CHP, US Coast Guard, LIFEFLIGHT and many others around the world. In 1982 Forman created 4MN Productions where he acted as both executive producer and host of ON SCENE: EMERGENCY RESPONSE?, ‘sPARKS?, ?FIRE RESCUE?, and ‘sPECIAL ACCESS?. His admiration for what he called the ?unsung hero’s? never faltered and was the catalyst for most of the shows 4MN Productions created from the early 80’s on.

He grew up in New York where his love of media began as a young man watching his uncle produce radio commercials for a major ad agency. His career began as a disc jockey and program director in the New York market. Soon after marrying he and his bride left for California where they have lived since. Forman moved quickly into management starting as the program manager of KEZY, Anaheim, then to radio and TV management at Group W/Westinghouse Broadcasting, including Executive Editor at the all news station, KFWB, Los Angeles.

In addition to his broadcasting career, Dave Forman wrote numerous articles for both print and web-based magazines, was the author of the non-fiction ‘scenes of Help?, and made a 2002 run for US Congress.

Forman, a devoted family man is survived by his wife, Barbara and their two children.

    Our prayers and thoughts are with you. I trust the wonderful memories of your husband and father will sustain you during this difficult time. My memory of Dave is a kind, gentle man who cared about family, his spirit will remain in you forever.

  4. My name is Paula Hernandez, I met both Dave and Barbara many years ago, at California First Bank, where I worked.

    I am very sorry of Dave’s passing and just now being able to express it, but I never forgot either one of you, You were both so charismatic, and very, very nice.

    Dave was certainly ahead of his time!
