Partnering with Hospice in Long-Term Care
Most residential care administrators and staff have had experience with hospice with one or more residents. Though these experiences are generally reported to be great, the hospice experience is still often shrouded in mystery. In this fast-paced workshop, long term care professionals are introduced to the official tenets for excellent hospice care set out by Medicare and that should be followed by all hospices providing care to your residents. Moreover, participants will learn how hospice professionals see their role in caring for patients at end-of-life, how hospice goals of care intersect with the protocols and practices of the long term care facility, and what to expect from hospice staff providing care within the facility.
Objectives. At the conclusion of this course, participants will be able to. . .
- Explain the background of hospice and how it provides care to dying persons and their families
- Identify the required components of hospice care required by Medicare rules
- Describe how hospice support and follow-up programs can enhance care to residents and their families, both during the final illness and after the resident’s death
